Covington, Newport, and Louisville chartered February 28, 1918 — just sixteen days after the IAFF itself was founded, and twenty-nine years before the Kentucky Professional Fire Fighters state association.

IAFF Local 38 of Covington, chartered on February 28, 1918, stands as one of the oldest fire fighter union locals in the United States. The Covington Fire Department itself was organized as a professional service in 1910 under first Fire Chief J.T. Swann. For over a century, Covington's fire fighters have maintained this proud union tradition of service to Kenton County.

IAFF Local 45 of Newport was chartered on February 28, 1918, the second of three Kentucky locals chartered that historic day. Built upon a fire department with roots reaching back to 1854 — paid professional service since June 9, 1868 — Newport's union brought organized labor protections to experienced fire fighters. The local withdrew in 1928 amid internal dissension, reorganized as Local 386 in 1934, and on March 14, 1995, the membership voted to reclaim the original Local 45 designation, restoring continuity with their 1918 founding.

Louisville fire fighters first petitioned to join the IAFF on January 1, 1917 — predating the IAFF's own official founding. Forced to withdraw under city opposition that made union membership grounds for dismissal, they re-chartered as Local 345 in 1941. In May 2022, nearly 104 years after their original 1918 petition, Louisville's membership voted to restore the original Local 54 designation, reclaiming their place in the IAFF's earliest history.
